Patient Eligibility

The Good Shepherd Clinic provides free medical care to residents of Clayton County, GA who have little to no access to necessary healthcare services. Patients must be medically uninsured, low-income, and at least 18 years of age. View our checklist to verify your eligibility.

Eligibility Checklist

To qualify for our services, you must be:

  • Clayton County resident
  • Medically uninsured (including Medicaid, Medicare & private insurance)
  • Age 18- 64 years*
    *Some exceptions apply; for example, if you do not qualify for Medicare
  • Have an annual Income at or Below 200% of the Federal Poverty Level
    ** See below for FPL guidelines

    Q: What do I need to bring to my appointment?

    A: You must bring the following to your appointment:

    • Proof of Clayton County residency: lease, mail, or utility bills with your name and address, etc.
      *Note: your ID is not considered an acceptable proof of residency
    • Proof of Income: pay stubs, letter of government benefits, tax return, notarized documentation of financial responsibility, etc.
    • Identification Card​

    Please bring your current medication bottles with you to your appointment, if possible.

    Q: What is the Federal Poverty Level, and how can I determine if I’m eligible?

    A: Federal Poverty Level is a measure of income issued every year by the Department of Health and Human Services. Federal poverty levels are used to determine your eligibility for certain programs and benefits.
